¿Qué es el esguince de tobillo?

El esguince de tobillo o torcedura de tobillo es una condición caracterizada por el desgarro o estiramiento de los ligamentos que estabilizan la articulación del tobillo (ver anatomía del tobillo). El ligamento colateral externo se lesiona con mayor frecuencia.

Un ligamento es un tejido fuerte (tejido conectivo) que mantiene unidos los huesos. El ligamento externo consta de tres haces y se encarga de conectar los huesos del peroné, el tobillo y el talón (ver anatomía del tobillo). El ligamento lateral externo trabaja de forma protectora para que no haya una pronación excesiva del labio medial (imagen 1). Cuando este movimiento se realiza en exceso, la articulación no resiste y se lesiona. Esta condición se llama esguince de fascitis plantar y varía desde un esguince simple (grado 1), desgarro parcial (grado 2) o desgarro completo (grado 3).

Causas del esguince de tobillo
Los esguinces de tobillo generalmente ocurren durante actividades que requieren cambios repentinos en la dirección del cuerpo, especialmente en superficies irregulares. Suele darse en jugadores de baloncesto, fútbol y voleibol. esto sucede cuando el pie se encuentra en la combinación de dos movimientos en sus posiciones extremas (extensión del labio medial, foto 1 y flexión plantar foto 2). En estas dos fases, la aplicación de una carga pesada (peso, caída de un salto, etc.) genera la lesión.

La fisioterapia se considera esencial para todos los pacientes con esguinces de tobillo porque no solo logrará una recuperación más rápida sino que también ayudará a prevenir una nueva lesión.
El tratamiento incluye:
• Crioterapia o termoterapia
• Movilización articular
• Técnicas de tejidos blandos (masaje)
• Electroterapia
• Corrección de la postura (si se considera necesario)
• Vendajes
• Ejercicios para mejorar la elasticidad muscular
• Ejercicios para mejorar la fuerza muscular
• Ejercicios para mejorar el equilibrio
